What is DDS?

The Massachusetts Department of Developmental Services (DDS) is the state agency responsible for providing supports and services to eligible individuals with intellectual and developmental disabilities.

What is a "Service Coordinator"?

A DDS Service Coordinator is the person assigned to help an individual plan supports, coordinate services, and connect with resources. The role is described in the DDS regulations covering Individual Support Planning under 115 CMR 6.00.

What is a "support plan" or ISP?

The Individual Support Plan (ISP) is the DDS planning document that describes a person's goals, services, and supports. ISP development is governed by 115 CMR 6.00 and is done with the person, family or guardian, and the DDS team.

Where does day-program transportation information come from?

Non-emergency transportation to MassHealth-covered services (such as Day Habilitation) goes through the MassHealth Prescription for Transportation (PT-1) request, now submitted online by the member's provider and coordinated by the Human Service Transportation (HST) program. HST assigns one of two regional brokers — MART or GATRA — depending on the member's city or town. DDS may separately fund transportation tied to a person's service plan.
